          This website is created and maintained by Oakland Community Heath Network (“OCHN”). By using this website you agree to the terms contained within this Privacy Policy.

          This Privacy Policy describes the information OCHN collects from you when you visit its website and how we handle that information. By accessing any of OCHN’s website or applications that reference or link to this Privacy Policy, including oaklandchn.org (“Site”) or by using any services we make available on the Site or that otherwise reference or link to this Privacy Policy (“Services”), you agree to be bound by the terms and conditions of this Policy.

          We may add to, modify, or otherwise update this Privacy Policy from time to time in our sole discretion by notifying you that a new version of this Privacy Policy has been posted to the Sites or Services. It is your responsibility to review this Privacy Policy for any additions, modifications or updates each time that you access any of the Sites or use any of the Services, and you are bound by such additions, modifications and updates. Your use of the Sites or Services following any additions, modifications or updates to this Privacy Policy constitutes your acceptance of the new Privacy Policy.

          2. PRIVACY POLICY

          Information collected from any user of OCHN’s Site or Service shall be used or disclosed for official purposes only, or as necessary to comply with any state or federal law. OCHN will adhere to the mandates of applicable state and federal law including, Health Information Portability Access Act (“HIPAA”), Michigan Internet Privacy Protection Act, Social Security Number Privacy Act National Crime Prevention and Privacy Act, Child Identification and Protection Act,. OCHN also recognizes and respects all state and federal law, which may require OCHN to disclose certain information. They include requests made under the Michigan Freedom of Information Act (“FOIA”), subpoenas, court orders, or any other applicable state or federal law.

           

          3. INFORMATION WE COLLECT

          In connection with the Sites and Services, we may collect the following categories of information:

          • information you provide to us, including, but not limited to, your name and your contact information (e.g., email address, phone number, postal address) o;
          • information regarding your use of the Sites and Services;
          • any other information that you provide or otherwise make available to us in connection with your use of the Sites or Services.

          You may choose not to provide certain information to us, but as a result you may not be able to take advantage of some of the features of the Sites or Services.

          4. TECHNOLOGIES USED TO AUTOMATICALLY COLLECT INFORMATION

          As mentioned above, we may use cookies, log files and similar technologies to collect certain information regarding your use of the Sites and Services.           5. Use and Maintenance of Personal Identifying Information

          OCHN and the company that hosts the website (“Host”) will retain personal information indefinitely, retention depends on the type of data and the purpose for which OCHN or Host process the data, the subscription term for any products which interact with the website, open records laws, and direction for the retention or deletion of such data. The Host may retain personal information beyond its contract with OCHN if required by law, contract, or if it is it their legitimate business interests and not prohibited by law.

           

          Collected information may be used in the following manner:

          • Deliver the services requested, including software updates; manage user relationships, provide users with customer support, and to comply with laws or regulations which apply to OCHN or Host.
          • To understand how users as a group use the website, resources, and services provided on the Site. OCHN or Host may use information to perform research and analysis about users use of, or interest in, the website.
          • The Host website may use information to improve its internal operations, systems, solutions, and services including benchmarking system performance and developing their products and services.
          • The Host will use the information to help respond to comments and questions and provide customer service, communicate with users and OCHN about products and/or services that may be of interest.

           

          6. PERSONALLY IDENTIFIABLE INFORMATION (“PII”) VOLUNTARILY SUBMITTED TO OCHN AND OTHER INFORMATION COLLECTED BY OCHN

          PII is information that can be used to distinguish or trace an individual’s identity, either alone or when combined with other information that is linked or linkable to a specific individual.

          PII can include:

          • Sensitive data, such as medical, financial, or legal information;
          • Neutral information, such as name, facial photos, or work address; and
          • Contextual information, such as a file for a specific health condition that contains a list of treated patients.

          If you choose to provide OCHN with your PII—for example, by completing a “Contact Us” form, leaving a comment, sending an email, or completing a survey—OCHN may use that information to respond to your message and/or help us get you the information or services you requested. Submitting personally identifiable information (PII) such as name, address, telephone number, email address, etc. is voluntary and is not required to access information on our website.

          We retain the information only for as long as required by applicable state and federal law including information submitted electronically. However, the Host may retain the information for a longer period of time.

          OCHN may also automatically receive information when you visit the website that is not PII

          Certain PII may be collected from you in order to:

          • Complete a request for an online service; or
          • Register for an event hosted by OCHN.

          Unless the circumstance mandate that OCHN disclose such PII under any state or federal law, OCHN will limit the disclosure to authorized contractors and agents as necessary to confirm completion of any required trainings.

          You do not need to register for an account if you simply wish to view certain pages on OCHN’s site.
