          National Core Indicators (NCI)

          Oakland Community Health Network takes part in the National Core Indicators project. This helps us learn how well services are working for people we serve by looking at important areas like jobs, health, and daily life. We use this information to make services better.

          Supported Decision-Making

          Supported decision-making is a way to help individuals with intellectual and developmental disabilities make choices about their own lives with guidance and support from trusted people. Instead of appointing someone else to decide for them, individuals work with supporters to understand options, weigh outcomes, and express their own preferences while retaining legal autonomy.

          What is Self-Determination?

          Self-Determination is a person-centered approach that gives individuals with developmental disabilities more control over how they live, work, and receive services. It encourages people to design their own supports, set personal goals, choose providers, and make decisions based on their own strengths and priorities — leading to greater independence and participation in the community.
