OCHN's comprehensive Provider Organization and Professional/Practitioner searchable directories provide important contact information for our service delivery network. They are user-friendly resources designed to encourage informed decisions by individuals seeking support, as well as awareness about specialty services offered by Oakland County's public mental health system, contracted service organizations, and licensed professionals/practitioners. These online resources are also available in print upon request.
OCHN has a long-standing history of promoting independence, choice, and community inclusion for adults and children with intellectual/developmental disabilities, mental health concerns, and substance use disorders. Its mission to "inspire hope, empower people, and strengthen communities" reflects an unyielding belief in a "Valuable System for Valued People." Most individuals who receive services through OCHN's service network have Medicaid insurance coverage.

All providers in the OCHN network are always accepting new people served to help support. We make every effort to support choice in all areas of service, please recognize some providers or practitioners may be unable to meet your needs at a specific time due to capacity. OCHN makes every effort to support choice in all areas of service. Please recognize some providers or practitioners may be unable to meet your needs at a specific time due to capacity issues. OCHN does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.
Assistance
You are provided this information once a year or upon request. The Provider Directory is available to everyone on the OCHN website at oaklandchn.org. If you are in need of assistance using the Provider and/or Practitioner Directories, please email Customer Services or call at 800-341-2003. Customer Services can help provide instructions on how to find practitioner information including name, gender, discipline, specialties (if applicable) if they are accepting new people, languages the practitioner speaks, and their office locations. In addition, Customer Services can provide instructions for searching the provider directory (Facility Name, Location, etc.) and any other Directory questions that you may have. You can also request a hard copy of the directories, at any time, at no cost to you, and will be issued within five business days.
