Important Data Breach Notice

OCHN has announced an inadvertent disclosure of Protected Health Information (PHI). This breach is generally, an impermissible use or disclosure under the Privacy Rule that compromises the security or privacy of the protected health information.

OCHN takes privacy and the protection of health information very seriously and immediately contacted all organizations involved in this situation to request and verify the following actions occurred: 

•    The PHI was not used or shared by any of the receiving parties for any reason. 

•    All electronic PHI was deleted and/or immediately returned to OCHN. 

•    All hard copies of the PHI were immediately destroyed.

•    Individuals impacted are being contacted by OCHN.
