Complex Case Management
Complex Case Management (CCM) is an enhanced service that assists people in coordinating all of their immediate healthcare and community resource needs. The purpose of the online form is to determine eligibility for the complex case management program.

Honor Community Health
Honor Community Health is a nonprofit, 501(c)(3) community partnership offering complete, coordinated, high quality primary, mental and dental health care throughout Oakland County, Michigan. Services are open to all, regardless of insurance or residence .
Oakland Housing Link is a collaborative project between Community Housing Network, Inc. (CHN) and Oakland Community Health Network.
Valuable public mental health services are necessary to ensure that jails do not replace institutions as an acceptable option for non-violent offenders with a mental illness. When alternatives to incarceration are made available to people, repeat experiences in the criminal justice system are avoided and paths to recovery are created.
